  • Our "TEAM" of health professionals, under the request and ongoing direction of the patient's personal physician is available to bring health care services to homebound patients.

  • Home care includes teaching you to take care of yourself or members of your family.  Home care offers competent comprehensive health care for yourself and other members of your family thereby promoting continuity of care between the institution and your home.  It provides a source of skilled care to those who prefer to remain in their home and for whom such care is appropriate and adequate.  Patients are happier and improve more rapidly in the surroundings of home and family.

  • Everyone from infant to senior is eligible for these services in their own home if they require intermittent medical care with one or more of the services.

  • Anyone may request Home Care Services, the Physician, the patient, family and friends.

  • Approved services are covered 100% by Medicare and Medi-Cal.  Private insurance coverage varies with individual policies.  We do a thorough research of insurance coverage to process all claims.

  • Home Health services must be ordered by a Physician.

    The service must be reasonable and necessary in the treatment of the illness or injury.

    The patient must require a qualifying skilled intermittent service such as nursing care, physical therapy, occupational therapy, or speech therapy. Medical social services and home health aide services are covered only if the patient is receiving Skilled nursing care or Physical or Speech therapy.

    The patient must be homebound.

    The patient is an eligible Medicare beneficiary.

    When all of the above conditions and criteria are met, Medicare will reimburse for an unlimited number of visits.

  • Inability to leave home without a considerable and taxing effort.

    Physician certifies Homebound status.

    Homebound patient is allowed for brief and infrequent absences from home for non-medical reasons.

    Patient with psychiatric condition who is considered unsafe to leave home unattended is considered homebound.

    Need for supportive devices in conjunction with physical limitations considered.

    Homebound status is not met if the patient is able to safely drive a car or is frequently able to leave the home for social reasons.

  • The main reason for utilizing in-home care is getting assistance in the home to facilitate independent living. Ultimate Home Health Care provides services for a few hours a week or 24/7 and everywhere in between. We tailor our care to your specific needs.

  • We can start on your in-home care plan right away if needed. We’ll work with you to determine the type and frequency of care and then provide you with one of our licensed, experienced, and compassionate caregivers as soon as you’re ready.

  • Absolutely. We provide care to clients who are in hospitals, assisted living facilities, rehabilitation centers, and other arrangements. Wherever you are, our quality caregivers are happy to come to you.

  • Our in-home care services for seniors may differ depending on your location. However, we are committed to providing dependable, compassionate, and highly-skilled care at a fair rate. Please contact us so we can discuss your specific needs and provide you with the most accurate quote.

  • We know that in-home care services can be costly. If Medicaid is not an option for you, we encourage you to explore our Private Pay Program. To discuss your needs and potential payment solutions, please feel free to contact us.

  • Many Ultimate Home Health Care clients have found our in-home senior care services to be more cost-effective than living in a facility, though rates vary by area and specific needs. Regardless of cost, it’s difficult to put a price on the sense of independence and quality of life you get by staying in your own home. If you contact us, we can help you figure out the most affordable solution for you.

  • Medicare covers acute, intermittent, short-term skilled care, usually after a hospital stay. We come alongside the Medicare benefits to deliver any long-term custodial care that may be needed. In the absence of Medicare benefits, we can also provide the necessary assistance required for you or your loved one to remain at home.

  • Medicaid coverage depends on services rendered and other circumstances, though some services are eligible. For questions or additional information, please contact us and we can help you figure out what may be eligible for Medicaid coverage.

  • Some services from Ultimate Home Health Care are covered by veterans’ benefits. Many of our locations provide services under VA benefits such as Aid and Attendance. Please contact us for more information.
